PURPOSE of SERVICE DIRECTORY

The purpose of this service directory is to provide an overview of The Resource Center's services. Because so many services are available, individual services are not described in detail here. If you have questions, please call us. If you have a request that does not appear to fit a service category, call us anyway, and we will see what can be done.

LOCATIONS

Services are provided at sites in Ashville, Dunkirk, and Jamestown and in homes and other community settings. Sites for adult services include Dunkirk (186 Lakeshore Drive West and 51 East Third Street) and Jamestown {75 Jones & Gifford Avenue and 880 East Second Street) and in various home and community settings. Sites for children's services {ages 0-5) include Ashville (South Maple Street near Hunt Road), Jamestown (10 Prospect Street), at home, at a licensed day care center, and at a licensed family day care home.

Services described in this directory include Service Coordination; Health services; Day and Community Support Services; Residential Services; Rehabilitation, Training and Employment; Mental Health Services; Children's Services; Community Educational Services; and Volunteer & Employment Opportunities.

SERVICE COORDINATION

Service coordination encompasses social work, central intake/referral, and Home and Community Based Medicaid Waiver services. Service coordination seeks to optimize Resource Center and community resources through individualized referrals and plans. (716) 661-1557.

Health Services

Health services as listed below are offered for a variety of purposes at a variety of locations. All are open to the general public as well as to persons with disabilities.

DAY and COMMUNITY SUPPORT SERVICES

There are a number of adult day services and community support services available. Services include:

  • Day Habilitation - Provides independent living skills, training and support.
  • Senior Day Habilitation - Participation requires a diagnosis of a developmental disability and waiver status. Geared to senior citizens age 50 and over.
  • In-Home Residential Habilitation - Provides independent living skills in private residences.
  • Day Treatment - Provides comprehensive services to persons with disabilities who exhibit severe deficits in adaptive behavior.
  • Orthocare - Provides durable medical equipment and adaptive equipment. Available to the general public. Call 661-1505.
  • Traumatic Brain Injury Support Group - For families, friends and survivors of Traumatic Brain Injury. This group meets the fourth Tuesday of each month at 7:00 p.m. in the First Methodist Church at the corner of Lakeview and Buffalo Streets in Jamestown.
  • Family Support and Training - Support and training are provided for parents. siblings, and other family members.
  • Parent Support Group - For parents of persons with developmental disabilities. Meets monthly on Thursday mornings at 880 East Second Street, Jamestown. Call 483-2344.
  • Guardianship - Facilitates the guardianship process for persons with disabilities.
  • Advocacy - Provided to individuals with disabilities and their families to assist with legal rights, financial assistance, housing, employment, guardianship, and financial planning.
  • Senior Parent Program - Seniors share their parenting skills with parents who have a developmental disability. To become a Senior Parent, call 483-2344. To gain access to this service, call 661-1557.
  • Family Empowerment Program - Provides assistance to families who have a family member with a developmental disability living in their home. This funds goods and services not covered by other funding sources.
  • Recreation - Facilitates individual choices in recreational planning.

Residential Services

The Resource Center offers a variety of residential services to meet various needs.

  • Individual Residential Alternatives - Customized residential settings tailored to individual needs.
  • Family Care - Provides residential placement in the home of a family care provider.
  • Respite - Provides temporary residential settings for persons with disabilities to provide a break or relief for parents and guardians.
  • Supportive Apartments - Provides independent living at Benedict Apartments in Jamestown.
  • Community Residences - Provides supervised living situations in homes in or near Dunkirk Jamestown and Westfield.
  • Intermediate Care Facilities - Provides supervised living situations for persons who are medically involved. Facilities are located in or near Jamestown.
  • Residential Habilitation - Trained staff in daily living skills acquisition of persons with developmental disabilities. Services can be provided in-home in a Family Care Home Individualized Residential Alternative as well as on vacations or in the community.

Rehabilitation Training and Employment

The Resource Center offers rehabilitation training and employment in order to promote independence among persons with disabilities.

  • Vocational Evaluation - Provides evaluation and career guidance.
  • Long-term Assisted Employment - Work Centers located in Dunkirk and Jamestown.
  • Environmental Services - Provides janitorial and lawn crews for hire in the community.
  • Placement Services - Provides skills in job seeking and on-the-job training.
  • Supported Work - Provides work opportunities for persons with severe disabilities through the assistance of job coaches.
  • Return-to-Work - Provides medical coordination & vocational services for injured or ill employees to return them to gainful employment.
  • Driver Education and Training - Teaches persons with disabilities how to drive a motor vehicle.

Mental Health Services

The Resource Center is a major provider of mental health services in Chautauqua County.

  • Continuing Treatment - A group therapy model for individuals requiring ongoing mental health services. The Gateways program is in Dunkirk and the Passages program is in Jamestown. Call (716) 661-1557.
  • Psychosocial Clubs - Offer individuals with mental health diagnoses peer support, socialization and access to community activities. "Social clubs" operate from 3:00 - 9:00 p.m. weekdays and periodically offer weekend activities. Call Doors in Jamestown (661-1510) and Windows in Dunkirk (366-7660).

Children's Services

The Resource Center provides services to children with disabilities from birth to five years old. Services are provided to children and their families on a customized basis. These services are offered at sites in Ashville and Jamestown; at day care centers and pre-schools: and at home sites. Transportation is provided.

  • Service Coordination - Provides service coordination for Individualized Family Services Plan for early intervention, which is intended to assist children as soon as a disability can be diagnosed.
  • Evaluation and Assessment - Screens and evaluates children for eligibility for early intervention program and assesses what services are needed. Call 763-9619.
  • Educational Services - Provides a comprehensive educational program for eligible children.
  • Family Services - Provides family training support services and services for parents and siblings.
  • Clinical Services - Available are psychology, physical therapy, occupational therapy, speech and language therapy, audiology, nursing and vision services.

Community Educational Services

  • "I'm Taking Care Of Me" - A live presentation designed to teach the pre- school and kindergarten populations that they can accept certain responsibilities in order to reduce the incidence of environmental accidents which could potentially result in disability. Available to pre-schools, day care programs, kindergartens and other groups. Call 661-1549.
  • "A Healthy Future" - A live presentation to teach the teenage population that they can accept certain responsibilities in order to reduce the incidence of birth defects which could potentially result in disability. Available to schools and other groups. Call 661-1549.
  • Outreach - Customized in-service presentations on Resource Center programs to staff members at hospitals, skilled nursing facilities, government agencies, schools, not-for-profit organizations and other groups located in Chautauqua County or which refer consumers to Chautauqua County services. For Dunkirk/Buffalo area, call 366-3510. For Jamestown/Erie area, call 661-1485.
  • Speakers Bureau - Qualified, trained individuals available to speak on a variety of topics drawing on the expertise of Resource Center staff members. Available to community organizations. Call 661-1434.

Volunteer and Employment Opportunities

  • Volunteer Program - Volunteer possibilities for working with individuals with disabilities, pre-school to senior citizens. 483-2344.
  • Resource Development - Opportunities are available for volunteer involvement in fund raising events throughout the year. 661-1434.
  • Employment - Substitute, part-time and full-time employment in many environments. Training and benefits provided. 483-2344.
